What information do we collect?
Journey Forward collects information from you when you register to download the Survivorship Care Plan Builder.
When registering you are asked to enter your: name, e‐mail address, role in survivorship care, name of organization where you provide care, the number of potential Survivorship Care Plan Builder users in your organization (there are no restrictions), and how you learned about Journey Forward. You may, however, visit our site anonymously.
What do we use your information for?
The information we collect from you may be used in one of the following ways:
- To improve our website, tools and resources (We continually strive to improve our offerings based on the information and feedback we receive from you.)
- To improve customer service (Your information helps us to more effectively respond to your customer service requests and support needs.)
- To send periodic emails about product updates and Journey Forward news (You can opt out of these communications at any time.)
- To administer user surveys (From time‐to‐time we send user surveys. Participation in these surveys is completely voluntary and you may choose whether or not to participate and disclose any additional information. Survey information will be used for the purposes of monitoring or improving the use and satisfaction of the Survivorship Care Plan Builder.)
Note: If at any time you would like to unsubscribe from receiving future emails, we include detailed unsubscribe instructions at the bottom of each email.
How do we protect your information?
We take precautions to protect your information. When you submit personal information via the website, your information is protected both online and offline.
Only employees and vendors who need the information to perform a specific Journey Forward job (for example, customer service) are granted access to your information. The computers/servers in which we store your information are kept in a secure environment.

We do not sell, trade, or otherwise transfer to outside parties your personal information with the exception of trusted third parties who assist us in operating our website, conducting our business, or NearSpace Medical servicing you. Our web vendor also agrees to keep your information confidential and abide by the usage guidelines described above. We may also release your information when we believe release is appropriate to comply with the law, enforce our site policies, or protect ours or others rights, property, or safety.

Occasionally, at our discretion, we may include links to third party websites on our website. These third party sites have separate and independent privacy policies.
