Resources for Advocacy Groups
We want you to be aware of a program designed to help cancer Survivors called Journey Forward. This program seeks to empower patients to request and obtain Survivorship Care Plans, and to support you in advocating for improved post treatment care for cancer patients.
An article in the Journal of Clinical Oncology states that care after treatment is an essential but missing step. The Institute of Medicine indicates survivor care should help:
- Prevent recurrent and new cancers as well as other late effects.
- Intervene for symptoms that result from cancer and its treatment.
- Coordinate the work of specialists and primary care physicians to ensure that all of a Survivor’s health needs are met.
We know from research that most primary care physicians and cancer patients want a Survivorship Care Plan. Survivors need to know that they will keep getting the same level of care following active treatment. Journey Forward can support you in your efforts to provide that bridge of care.
